Semaglutide (injection) vs Rybelsus (oral): the same molecule, different delivery
Both are semaglutide. One is a weekly injection (Ozempic, Wegovy); the other is a daily tablet (Rybelsus). Here's why that matters and why it doesn't.
Semaglutide comes in two delivery formats: a once-weekly subcutaneous injection (Ozempic for T2D, Wegovy for obesity) and a daily oral tablet (Rybelsus for T2D only). The molecule is identical; the formulation, indication, dosing, and practical experience differ. Rybelsus uses an absorption-enhancing formulation called SNAC (sodium N-[8-(2-hydroxybenzoyl)amino]caprylate) to enable oral absorption of the peptide — otherwise it would be destroyed by stomach acid and gut proteases.

Is Rybelsus as effective as Ozempic?
At labeled doses (Rybelsus 14 mg vs Ozempic 1 mg), HbA1c reductions are comparable in T2D populations. Rybelsus's bioavailability is low (~0.4-1%), so the pill's mg dose is much higher than the injection's, but the clinical effect is similar at matched indications.
Can I use Rybelsus for weight loss?
It's not FDA approved for obesity (only T2D). Some clinicians prescribe it off-label for weight management, but insurance coverage typically requires the T2D indication. For labeled weight management, Wegovy (injection) is the semaglutide product.
Why do I have to take Rybelsus fasting?
The SNAC-enhanced absorption system requires minimal water and an empty stomach to work. Eating, drinking (other than sipping water), or taking other medications within 30 minutes significantly reduces absorption. Compliance with this regimen is the single biggest real-world challenge.
Is Rybelsus a pill version of Wegovy?
